Property Maintenance Request Form

A Property Maintenace Request Form can be submitted either by e-mail or fax. Click here for form.

First fill out the form, typing over the lines by turning your Insert mode off (i.e. your typing replaces the lines rather than inserting your typing before the lines) by hitting the Insert key.

Save the form by clicking on File at the top left of your screen, and then on Save As. Save the file in a folder where you can find it again. Hit your Insert key again to turn the Insert mode back on.

TO E-MAIL:
Click on File, then Send To, then Mail Recipient (as Attachment).  
Insert Loyd's e-mail address (roscowl@adorers.org) in the To field.
Add any additional information in the body of the e-mail.
Click on Send.

TO FAX:
Click on File, then Print.
Click on Print.
